A Comparison of Laparoscopic versus Open Appendectomy during Pregnancy

Abstract

|
Purpose: Although it has been reported that a laparoscopic appendectomy is as safe as open surgery, the safety of a laparoscopic appendectomy during pregnancy has not yet been established. Our study aimed to compare a laparoscopic appendectomy with an open appendectomy, and to ascertain the safety and feasibility of laparoscopic surgery in the management of acute appendicitis during pregnancy.
Methods: Between January 1999 and December 2006, 61 pregnant women with acute appendicitis underwent surgery at our hospital. There were 46 patients in the open appendectomy group (OA group) and 15 patients in the laparoscopic group (LA group). Patient data was evaluated for operation time, hospital stay, postoperative complications, use of tocolytics, and fetal outcome including abortion, premature delivery, birth weight, gestation duration, intrauterine growth retardation, congenital malformation and neonatal mortality.
Results: All laparoscopic appendectomies were performed without conversion to open surgery. There were no significant differences between the two groups for operation time, a patient undergoing an abortion, premature delivery, birth weight, and presence of a congenital malformation. Tocolytic medication was not administered to any patient in the LA group. Hospital stay was much shorter, and the postoperative complications were not noted in the LA group of patients.
Conclusion: For appendicitis during pregnancy, a laparoscopic appendectomy seems to be safe for the mother and fetus, and stable results were indicated in the third term of pregnancy if the operation was performed carefully. If prospective randomized controlled trials are performed for a larger population, statistically significant results are expected.
